Who We Are
Cendity Health is an independent family medicine practice in downtown Northampton, Massachusetts. We are not part of a hospital chain or a large health system. We provide primary care to patients of every age, from newborns to grandparents, from our office at 214 Bridge Street.
Dr. Susan M. Reyes opened Cendity Health in May 2026 with a simple goal: to offer the kind of steady, personal primary care that a family can rely on for years. That goal still guides how we work. We are here for the whole family, and we look forward to caring for our patients — and one day their children — for years to come.
Our Approach to Care
Family medicine means we can care for the whole family under one roof and across a whole lifetime. The same practice that gives your child their checkups can manage your blood pressure, support you before and after a pregnancy, and help an older parent stay well at home.
We practice whole-person, relationship-based care. That means we get to know you, not just your chart. When we understand your history, your family, and what matters to you, we make better decisions together and catch problems earlier. A relationship built over years is the most useful tool we have.
The care we provide includes:
- Annual physicals and wellness visits for adults and children
- Well-child care, immunizations, and school and sports physicals
- Care for ongoing conditions such as diabetes, high blood pressure, asthma, and thyroid disease
- Evaluation of new symptoms and same-day sick visits when we can offer them
- Women’s health, including routine screenings
- Preventive care, screenings, and health counseling
- Coordination with specialists and local hospitals when you need care beyond our office
Our Providers
Our clinicians work as a team, and each brings full training in family medicine. When you need care and your usual provider is not available, you can see a colleague who has access to your records and knows how the practice works.

Our practice administrator, Joan Harmon, oversees the day-to-day running of the office. If you have a question about billing, records, or scheduling that the front desk cannot answer, she is glad to help.
Part of the Pioneer Valley
We are proud to be a local, independent practice. Northampton is our home, and we care for neighbors from across the Pioneer Valley, including Amherst, Easthampton, Holyoke, Hadley, South Hadley, and Belchertown. Our staff live in these communities, and many of our patients are people we see at the market, at school events, and around town.
What Independent Means for You
Being independent means the people who care for you also run the practice. Decisions about how long a visit lasts, how we follow up, and how we treat patients are made here, by our own clinicians, not set by a distant corporate office.
In practice, that looks like a few things:
- You see familiar faces. We keep our team stable so you are not starting over with a stranger at each visit.
- We take the time a visit needs and know your history when you walk in the door.
- You can reach a real person on the phone during office hours, and you can use our patient portal to message us, view results, and request refills.
- Our loyalty is to our patients. That is the whole reason the practice exists.

Office hours are Monday through Thursday from 8:00am to 5:00pm and Friday from 8:00am to 4:00pm. We are closed Saturday and Sunday. If you have a medical emergency, call 911.
