Welcome to Cendity Health

Thank you for considering Cendity Health for your care. We are a new independent family-medicine practice in Northampton, caring for patients of all ages across the Pioneer Valley, including Amherst, Easthampton, Holyoke, Hadley, South Hadley, and Belchertown. Our office is at 214 Bridge Street, Suite 200, in Northampton.

Because we are a small, physician-owned practice rather than part of a large hospital system, you will get to know your provider and our staff, and they will get to know you. We look forward to being your medical home.

How to become a patient

Getting started is simple. Call our office at (413) 586-2740 and let us know you would like to establish care as a new patient. Our staff will confirm that we are accepting new patients, check that we participate with your insurance plan, and schedule your first appointment with one of our providers.

When you call, please have your insurance card handy so we can record your plan and member information. If you are transferring from another practice, let us know the name of your previous provider so we can request your records.

Before your first visit: new-patient forms

We ask every new patient to complete a short set of forms before the first appointment. These cover your contact information, medical and family history, current medications, allergies, and insurance details. Completing them ahead of time means more of your appointment can be spent talking with your provider.

There are two ways to complete your forms:

  • Online through our patient portal at portal.cendity.com once you have registered (see below). This is the fastest option.
  • On paper at the office. If you prefer to fill out your forms in person, please plan to arrive about 20 minutes before your scheduled appointment time so you have time to complete them at check-in.

What to bring to your first appointment

Please bring the following to your first visit:

  • A current photo ID, such as a driver’s license or state ID
  • Your insurance card (and any pharmacy or prescription benefit card)
  • A list of all medications you take, including the dose and how often, along with any vitamins or supplements
  • Copies of relevant medical records, immunization records, or recent test results, if you have them
  • The name and address of your pharmacy
  • Any co-payment your insurance plan requires, which is collected at check-in

If you would like us to obtain records from a previous provider on your behalf, please give our front desk the practice name and phone number and we will send a records request.

What to expect at your first appointment

Your first visit is a chance for you and your provider to get to know each other. Please arrive a few minutes early to check in. During the appointment, your provider will review your medical history, current medications, and any concerns you have, and will talk with you about your health goals. Depending on your needs, the visit may include a physical exam and orders for routine screenings or lab work.

Plan to spend a little more time with us than at a typical follow-up visit, since we are gathering a complete picture of your health. Please bring any questions you have written down so we can address them together.

Register for the patient portal

Our patient portal at portal.cendity.com lets you complete new-patient forms, message our office, request appointments and prescription refills, and review your visit summaries and test results at any time.

To register, provide your email address when you schedule your first appointment or when you call the office. We will send you a secure invitation with a link to create your account and set a password. If you have already scheduled a visit and have not received an invitation, or if you need help signing in, call us at (413) 586-2740 and our staff will assist you.
