Cendity Health is a new independent family medicine practice in downtown Northampton. We care for patients of all ages — from newborns to seniors — and we build long-term relationships so that the people looking after your health actually know you. Our team includes physicians Susan M. Reyes, MD, Daniel C. Okonkwo, MD, and Rebecca S. Feldman, DO, along with Family Nurse Practitioner Karen A. Whitfield, FNP-BC, and Physician Assistant Marcus T. Bell, PA-C.

Primary & Preventive Care

Preventive care is the heart of a family practice. We work with you over time to keep you healthy, catch problems early, and manage risk factors before they become serious.

  • Annual physicals and wellness visits — a yearly check-in to review your overall health, update your medications, and set goals together.
  • Medicare Annual Wellness Visits for our older patients, including a personalized prevention plan.
  • Health screenings for blood pressure, cholesterol, diabetes, colorectal and other cancers, osteoporosis, and depression and anxiety.
  • Well-child and school, camp, and sports physicals.
  • Lifestyle and prevention counseling — nutrition, weight, exercise, tobacco cessation, and alcohol use.
  • Pre-operative clearances and physicals for work, DOT, and other requirements.

Chronic Disease Management

Ongoing conditions are easier to live with when someone is helping you stay ahead of them. We provide steady, coordinated care for long-term health concerns and adjust your plan as your needs change.

  • Diabetes — monitoring, medication management, and support with diet and blood sugar control.
  • High blood pressure (hypertension) — regular monitoring and treatment to protect your heart and kidneys.
  • Asthma and COPD — breathing assessments, inhaler technique, and action plans.
  • Thyroid disorders — evaluation and management of an underactive or overactive thyroid.
  • High cholesterol, heart disease risk, and metabolic conditions.
  • Care coordination with specialists and hospitals across the Pioneer Valley when you need more than office care.

Acute / Sick Visits

When you wake up feeling unwell, you want to be seen by people who know you. We keep same-day appointments open when possible for sudden illnesses and minor injuries.

  • Colds, flu, sore throats, sinus infections, and coughs.
  • Ear and eye infections, urinary tract infections, and skin rashes.
  • Minor injuries, sprains, cuts, and burns.
  • Stomach upset, fevers, and other new symptoms.

Immunizations & Travel

We keep routine vaccines on hand and help you plan ahead for trips abroad.

  • Childhood and adolescent immunizations on the recommended schedule.
  • Adult vaccines, including seasonal flu, tetanus (Td/Tdap), shingles, pneumonia, and COVID-19.
  • Travel consultations with advice on vaccines and health precautions for your destination.

Women’s Health

We provide routine gynecologic and reproductive care as part of your regular family medicine visits, so you can address these needs with a provider you already trust.

  • Well-woman exams and Pap smears.
  • Birth control counseling and prescriptions.
  • Menopause care and management of related symptoms.
  • Breast health, cervical cancer screening, and referrals for mammography.
  • Preconception counseling and evaluation of common concerns.

Pediatric & Adolescent Care

As a family practice, we care for the whole household — including your children and teens — from infancy through the college years.

  • Well-child visits and developmental checkups.
  • Immunizations and school, camp, and sports physicals.
  • Sick visits for common childhood illnesses.
  • Adolescent health, including confidential conversations about growth, mood, and healthy choices.

Geriatric Care

Older adults often manage several conditions and medications at once. We take the time to look at the full picture and support both patients and their families.

  • Management of multiple chronic conditions and medication reviews.
  • Memory, mobility, balance, and fall-risk assessments.
  • Medicare Annual Wellness Visits and preventive screenings.
  • Advance care planning and coordination with home and community services.

Minor Office Procedures

Many small procedures can be handled right here in our Northampton office, saving you a trip to a specialist or hospital.

  • Skin biopsies and removal of moles, cysts, skin tags, and warts.
  • Laceration repair (stitches) and treatment of minor wounds and abscesses.
  • Joint injections for pain and arthritis.
  • Ear wax removal and toenail procedures.

On-site Lab Work

We offer blood draws and specimen collection in the office, so much of your routine testing can be done during your visit.

  • Blood draws for common lab tests, with results reviewed by your provider.
  • In-office rapid testing for strep, flu, COVID-19, and urinalysis when appropriate.
  • Test results and follow-up shared securely through our patient portal at https://portal.cendity.com.

Telehealth

Some visits do not require coming into the office. Telehealth lets you connect with your Cendity Health provider by secure video from home or work.

  • Follow-up visits for stable chronic conditions.
  • Medication reviews and refill discussions.
  • Minor concerns that do not need a hands-on exam.
  • Discussion of lab and imaging results.

Your provider will let you know when a telehealth visit is a good option, or you can ask when you call to schedule.

We welcome patients throughout Northampton and the wider Pioneer Valley, including Amherst, Easthampton, Holyoke, Hadley, South Hadley, and Belchertown. To schedule, call our office at (413) 586-2740 or send a message through the patient portal at https://portal.cendity.com.

Our office hours are Monday through Thursday, 8:00am to 5:00pm, and Friday, 8:00am to 4:00pm. We are closed Saturday and Sunday. For a medical emergency, call 911.
